UnitedHealthcare Awards $30,000 Grant to Heartland Family Service

The Heartland Family Service Integrated Health Home program received a $30,000 grant from UnitedHealthcare in a presentation on Monday, November 23. The grant will help women and families connect with health care, prenatal care, health education and other programs that include peer support coordination and preventative care services.

The Heartland Family Service program provides integrated care coordination for adults with a severe mental illness. Heartland Family Service, Alegent Creighton Health, Children’s Square, U.S.A. and Magellan Behavioral Care of Iowa are working together to establish innovative strategies to provide holistic care that focuses on wellness. A team of IHH staff, including a care coordinator, nurse care manager and peer support specialist, provide mental health and physical health care coordination to ensure members are accessing the medical, physical, and social services they need. Heartland Family Service provides care to eligible individuals in Pottawattamie, Harrison and Mills counties in Iowa.
