Douglas County Health Center – Where the Spirit of Care Comes to Life
Douglas County Health Center (DCHC; dchc.douglascounty-ne.gov) has proudly served the Omaha area for over 100 years and continues to be a leader in health care in the community.
Douglas County Health Center’s team of health care professionals provides excellent short-term, post-acute rehabilitation, and long-term care. They are recognized by the Centers for Medicare and Medicaid as a five-star-rated facility, and take pride in being the only facility in Nebraska to hold that rating since it’s inception in 2008.
Rehab Services
At Douglas County Rehab, individuals stay in one of the 20 modern, private rooms in the short-term unit. All short-term stay individuals at Douglas County Rehab enjoy complimentary transportation to medical appointments, customized meals, recreational services, rehabilitative services, such as speech, physical, and occupational therapy, and individual’s care planning initiated within 24-hours of admission.
Physical Therapy—The DCHC physical therapy team provides each patient with a comprehensive evaluation that allows them to create an individualized treatment plan that helps each patient return home active with increased muscle strength and endurance. Patients receive daily private one-on-one therapy sessions to help them improve upon their balance and coordination following a major surgery, injury, or illness.
Occupational Therapy—DCHC uses task-oriented activities to help patients re-gain their functional abilities that have become impaired due to illness, injury, or aging. Each patient has an individualized occupational therapy plan that is designed to help each patient adapt to their social and physical environment by mastering tasks essential for daily living.
Speech Therapy—The speech-language pathology team at DCHC can provide diagnostic and therapeutic speech therapy services. Our team of experts can work with patients who have cognitive, language, speech, swallowing, and voice disorders due to stroke, brain injury, dementia, neurological disorders, hearing impairment, respiratory illnesses, and many other complex medical conditions.
Memory Care
The DCHC staff understands that caring for someone with Alzheimer’s or Dementia can be challenging. When care can no longer be provided at home, the staff at Douglas County Health Center invites you to entrust your loved one’s care to them. The Memory Care Program creates a comfortable and secure environment that both the individual and their family will enjoy. Memory care services include:
- In-house physician and 24-hour professional nursing staff
- Secure environment
- Individualized health monitoring
- Medication management
- Personalized assistance with daily needs
- Delicious meals and snacks are always available
- Daily interactive and socially stimulating activities designed for those with memory loss
Respite Care
The Douglas County Nursing Community offers Respite Care services for families who need a break from their at-home caregiving routine. The DCHC dedicated staff provides around-the-clock nursing supervision for your loved one in a safe, nurturing environment. Additional features of this program include: variable length of stay schedules, medication management, rehabilitation, and clinical care services as needed.
Activities
DCHC employs outgoing and friendly professional staff to create social programs and activities, designed to match the capabilities and needs of each resident. This has a powerful impact on the healing process. Each resident is given a calendar to keep them informed of upcoming events, such as birthday parties, movies, musical programs, games/trivia, gardening, volunteer involvement, and other special events. Friends and families are always welcome to visit and attend our on-site events.
